Below are the details for the passport document DS-11. You are required to download and fill it out when applying for a new passport in person.

What to submit with form DS-11

Below is a list of documents you will need to send along with passport form DS 11.

Proof of US citizenship

Evidence of US citizenship and a photocopy of the front (and back, if there is printed information) must be submitted with your application. The photocopy must be on 8 ½ inch by 11 inch paper, black and white ink, legible, and clear.

Evidence that is not damaged, altered, or forged will be returned to you. Please note, lawful permanent resident cards submitted with this application will be forwarded to US Citizenship and Immigration Services, if they determine that you are a US citizen.

Proof of your identity

You must present your original identification and submit a photocopy of the front and back with your passport application.

How to submit passport form DS-11

You will need to complete and submit this application in person to a designated acceptance agent

  • A clerk of a federal or state court of record or a judge or clerk of a probate court accepting applications
  • A designated municipal or county official
  • A designated postal employee at an authorized post office
  • An agent at a passport agency (by appointment only), or
  • A US consular official at a US Embassy or Consulate, if abroad

Passports for children under 16

If you are applying for a passport for a child under the age of 16, then see the passport for children under 16 page for further details on what is required.

Passport validity length

If the applicant is 16 years of age or older then your US passport will be valid for 10 years from the date of issue except where limited by the Secretary of State to ashorter period.

If the applicant is under the age of 16then your US passport will be valid for five years from the date of issue except where limited by the Secretary of State to a shorter period.

Applicants with previous passport book and or card

If your passport was lost or stolen and you cannot submit your valid or potentially valid US passport book and/or passport card with this application and you have not previously submitted Form DS-64, Statement Regarding a Lost or Stolen US Passport, you are required to fill out and submit a DS-64 along with form DS-11.

If you have your passport with you, then if your most recent US passport book and/or passport card was issued less than 15 years ago, and you were over the age of 16 at the time of issuance, you may be eligible to use Form DS-82 to renew your passport by mail. Please note, if you fail to provide information requested from passport form ds 11, then it could cause a significant processing delay and or denial of your passport application.
